Original 1st printing 6&1/8 x 6&1/8" uncut sheet of TEN (10) 1 x 3" jukebox insert paper tabs for the 1974 UA single of "U.S. Blues/Loose Lucy" by the Grateful Dead.
 
Medium-thin index stock sheet (with perforations) is in excellent (B+) condition; aside from a small staple hole wedge pull in the top center margin and a couple of handling dings, it is a VERY sharp example of this oddball little mid-'70's Grateful Dead artifact from who the heck knows where?
